Why we ask for Parental Consent:

In caring for the best interest of your child, it is important that the County is aware if they suffer from any illness or medical condition and if they are currently receiving medical treatment of any kind.

Please indicate on the consent form if this is the case.  Details of any prescribed medicine with dosage, special dietary requirements and allergies are also requested.

Please note that before taking part in any County event, training or representing Warwickshire, you must have completed and submitted this form below .

Notifying us of changes:

If any medical or contact details change throughout 2023 you must let us know by completing a new form and submitting to us so we can keep our records up to date.

Photograph / Video Policy:

The County recognises the need to ensure the welfare and safety of all children in golf. As part of our commitment to ensure their safety we will not permit photographs, video or other images of your child to be taken or used without your consent. The County will take steps to ensure that these images are used solely for the purpose for which they are intended i.e. the promotion and celebration of the activities.

Data Protection Policy:

Please note the information supplied above will be kept in strictest confidence on a database and only be used by Committee members of the County for the purposes of organising competitions, coaching, matches or other activities for the Junior Section (including results published in the press) and will not be disclosed to any third party.

Changing Room and Toilets Policy:

The changing rooms and toilets at all Golf Clubs we visit are used by all members and visitors to the Club. When children are attending any event / coaching sessions parents should be made aware that adults use the changing rooms and toilets throughout the day for changing and showering. If as a parent / guardian do not consent to your child accessing the changing rooms, it is your responsibility as parents / guardians to either supervise the child while in the changing rooms or ensure that they do not use them.
